Speak and gesture at the same time

Real talk mixes words with nods, waves, and shrugs. Stroke and ALS can take both. Older brain implants often restored speech or movement — not both in the same breath. On 14 September 2026 Nature Neuroscience published Brosler, Liu, Silva et al. (Chang Lab, UCSF): one implant on the surface of the brain decoded attempted speech and gestures together, with two models running in parallel, driving a personalized full-body avatar. What happened

Brosler et al. recorded from people with severe paralysis using a high-density grid of sensors on the surface of the brain (under the skull), over the area that helps plan speech and body movement. Isolated arm and face movements could be decoded. With parallel speech and gesture models, participants controlled a personalized virtual avatar while attempting speech and gestures at the same time or alone. Training on both isolated and simultaneous attempts worked better across contexts.

Coverage adds color. New Scientist reported roughly 88% gesture / 84% speech accuracy for one stroke participant and about 66% / 70% for one ALS participant on tested phrase–gesture pairs. A Nature Portfolio press note said one conversational block set reached median 100% for both speech and gesture in one participant across three blocks. Nature news noted on the order of 253 electrodes on the cortical surface.

Here is the sharper scoreboard. What is narrowly new is simultaneous speech plus gesture decoding from one surface-of-brain implant, with parallel models driving a personalized avatar in real time — and evidence that training must include simultaneous attempts. Speech-only and gesture-only implants already existed. This is not home daily use for everyone, not unlimited vocabulary, and not proof the person’s own voice and body already work.

Honest limits

  • Phrase and gesture sets are limited — on the order of tens in the reported tests, not open conversation.
  • Few participants (three for isolated decoding; avatar control highlighted for two in coverage).
  • Accuracy varies by person; independent comment says higher accuracy is needed for daily use without frustration.
  • Avatar plus on-screen text prove dual decoding — they are not the person’s own voice and body restored today.
  • This is an implanted medical-device pathway — not a wearable app.
  • Authors note the need for more participants and larger word/gesture sets before practice use.
  • Naming a lab or brand is not the invention.
